These Terms and Conditions (“Terms”) govern your use of TruForma Health’s website and related services (collectively, the “Service”). TruForma Health is a DBA of TruForma Health LLC (“TruForma Health”, “we”, or “us”).

The Platform allows users to connect with licensed healthcare providers through telehealth consultations.

Healthcare providers using the Platform are independent medical professionals who are responsible for the medical care they provide.

TruForma Health serves solely as a platform and facilitator connecting you with licensed healthcare providers (the “Providers”). TruForma Health (TruForma Health LLC) itself is not a medical provider, does not practice medicine, and does not directly provide medical services or medical advice.

Information provided on the Platform is not intended to replace professional medical advice, diagnosis, or treatment.

By accepting these Terms, you acknowledge and expressly agree that TruForma Health shall have no liability or responsibility for medical services provided by Providers, including diagnosis, treatment decisions, prescriptions, or medical advice. You expressly waive and release any claims against TruForma Health relating to such services.

You must be at least 18 years old to use the Platform.

By using the Platform, you represent that you meet this requirement.

TruForma Health does not participate in federal or state healthcare programs such as Medicare or Medicaid. You agree not to submit claims to insurance or government healthcare programs for reimbursement.

Users agree to pay all fees associated with services received through the Platform.

Payment processing may be handled by third-party payment processors. We use reputable third-party payment processors that are required to maintain appropriate administrative, technical, and physical safeguards.

Certain products and services are provided on a subscription basis. Your subscription will automatically renew unless canceled. You may cancel or modify your subscription through your online account or by contacting our Customer Care Team.

Certain services require prescriptions from licensed Providers. Prescriptions will be filled either through our affiliated pharmacy partners or through a pharmacy of your choice, depending on the product. You may choose alternative pharmacies or laboratories and may request prescription transfers at any time.

You agree to use telehealth for consultations and services provided by TruForma Health Providers. You acknowledge that telehealth has inherent limitations, and certain conditions may not be suitable for diagnosis or treatment via telehealth. Providers retain full discretion regarding treatment suitability. You consent to receive electronic communications, including emails and portal messages, which may contain your health information (PHI). You understand these communications are not fully secure and accept the potential risks of electronic transmission.

Your privacy is important. Certain health-related information provided through our Service is protected by federal and state privacy laws (e.g., HIPAA). Such information will be handled securely and in compliance with applicable laws, as detailed in our Privacy Policy. You are responsible for maintaining the confidentiality of your account information and password and for restricting access to your account. Notify us immediately of any unauthorized use of your account.

TruForma Health owns all rights to the Service’s content, branding, and intellectual property. You are granted a limited, non-transferable license to use the Service strictly for personal use. Unauthorized use or reproduction is strictly prohibited.

You agree to indemnify and hold TruForma Health and its affiliates harmless from any claims arising from your misuse of the Service or violation of these Terms.

TruForma Health reserves the right to terminate your access to the Service at any time, without notice, for violations of these Terms or for any reason deemed necessary by TruForma Health.

To the fullest extent permitted by law, TruForma Health shall not be liable for indirect, incidental, or consequential damages arising from the use of the Platform. Our liability, if any, shall not exceed $1,000.

We reserve the right to update these Terms at any time. Continued use of the Platform after changes are posted constitutes acceptance of the updated Terms.

Unless you opt out, disputes between you and us or between you and any medical providers associated with TruForma Health will be resolved by binding arbitration, and you waive the right to a jury trial and to participate in a class action lawsuit or arbitration.

Disputes shall be resolved by confidential arbitration administered by the American Arbitration Association (“AAA”) in Wyoming, under its Consumer Arbitration Rules. You may opt-out within 30 days of acceptance by contacting our Arbitration Opt-Out department.

Governing Law These Terms shall be governed by the laws of the State of Wyoming.

Entire Agreement These Terms constitute the entire agreement between you and TruForma Health regarding your use of the Platform.
