By using telehealth services through this Platform operated by TruForma Health (a DBA of TruForma Health LLC), you acknowledge and agree to the following:

Risks of Telehealth

Potential risks include technical failures, incomplete information due to electronic communication and delays in evaluation or treatment.

Benefits of Telehealth

Benefits may include improved access to healthcare providers, convenience and flexibility and reduced travel time.

Emergency Situations

Telehealth is not suitable for emergencies or urgent care. If you experience an emergency or urgent medical condition, immediately call 911 or seek care from your local emergency provider.

Patient Responsibilities

You agree to:

Limitations and Consent

Open Payments Notice

In accordance with the federal Physician Payments Sunshine Act, you may review payments or transfers of value made to providers by pharmaceutical and medical device manufacturers through the federal Open Payments database: https://openpaymentsdata.cms.gov.

Financial and Pharmacy Disclosures

You understand and accept responsibility for all fees associated with telehealth services and medications. TruForma Health may have financial relationships with certain pharmacies or labs. You are free to choose alternative providers or pharmacies and may request prescription transfers.

Consent to Treatment

By using the Platform, you consent to receiving telehealth services from licensed healthcare providers.

You may withdraw consent at any time by discontinuing use of the Platform.

Contacting Regulatory Authorities

Concerns regarding medical care may be directed to your state’s medical board.
